General Notes

Data subjects and/or their legal representatives may submit their requests regarding their rights relating to the protection of their personal data in writing or by other means to Citio Dijital Teknoloji ve Ticaret Anonim Şirketi (“Citio” or the “Company”), acting in its capacity as data controller.

In this context, you may submit written applications to our Company through this form as follows:

• By applying to Citio in person together with a valid identity document,
• By post to our address “Akat Mah. Selçuklar Sk. Eti Apt No: 12 İç Kapı No: 6 Beşiktaş / İstanbul” with a wet signature and a copy of your identity document,
• By e-mail to Citio's address kvkk@citioapp.com with a petition signed with a mobile signature or a secure electronic signature.

If the application is made to our Company in person, the phrase “Concerning a Request Regarding the Protection of Personal Data” must be written on the application envelope; if made by e-mail, it must be written in the subject line.

Our Company may take the necessary measures and request additional information to verify the identity of the Applicant in applications made by the Applicant.

Your applications submitted to us will be answered, depending on the nature of the request, within thirty days from the date the request reaches us. The responses provided are notified to you in writing or electronically.

• I want to collect it in person. (In the case of collection by proxy, a notarised power of attorney or authorisation document is required.)

This application form has been prepared to determine your relationship with our Company, to fully identify your personal data processed by our Company, if any, and to enable an accurate and timely response to your application within the legal period. In order to eliminate the legal risks that may arise from unlawful and unfair data sharing and, in particular, to ensure the security of your personal data, our Company reserves the right to request additional documents and information (such as a copy of an identity card or driving licence) for identity and authorisation verification. In the event that the information regarding the requests you submit within the scope of the form is inaccurate or out of date, or an unauthorised application is made, our Company accepts no liability for requests arising from such inaccurate information or unauthorised applications.
